Steps to follow:

1. Email your stories and pictures to portcredit.arena@mississauga.ca

2. Download the Photography release for promotional use form

3. Complete the form.

PLEASE NOTE: Pictures of children under the age of 12 must have parents complete the form including all family names, first and last in the area provided.

4. VERY IMPORTANT: All people who can be recognized in a picture must give written permission (a signed form). The individual who is responsible for submitting the picture is responsible to have photography release forms signed by all third parties and returned together to Cammie LaCoste BEFORE the picture will be posted. In order to identify the picture submitted, all photography release forms must include (anywhere on the page) the email address of the individual who was responsible for submitting the picture.

5. Forms may be faxed to 905-615-3469 attention Cammie LaCoste or mailed to 201 City Centre Drive, Suite 900, Mississauga, ON L5B 2T4 attention: Cammie LaCoste. Your email address must be included on your completed photography release form(s) in order to match your picture with your or third party permission.
